Molina Healthcare is seeking an experienced Care Manager RN to join our care coordination team. You will assess members, develop and implement care plans, and collaborate with physicians, family, and other health professionals to improve outcomes.

This role involves telephonic, in-person, and home visits, with 25-40% travel. You will support integration of services across behavioral health, LTSS, and community resources while maintaining patient-centered communication and timely documentation.

Qualifications

• Active and unrestricted RN license in state of practice.
• 2+ years in healthcare, preferably care management or related setting.
• Valid driver’s license and reliable transportation.
• Understanding of EMR and HIPAA.
• Ability to work independently and with diverse populations.
• Excellent time-management and verbal/written communication.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with member, caregiver, physician and care team.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor and adjust care plans based on outcomes and interventions.
• Maintain ongoing member caseload and outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration.
• Conduct medication reconciliation as needed.
