Job summary

Humana Inc. in Indiana is seeking a Care Coordinator 2 (Field Care Manager 2) to assess and coordinate member needs under the Indiana PathWays for Aging program.

You will guide members and families toward resources to improve wellness and ensure comprehensive care coordination, including interfacing with Medicare payers and providers. The role requires Indiana-licensed RN with 2+ years of case management experience, strong communication, and proficiency in EMR systems.

Qualifications

• Licensed Registered Nurse (RN) in Indiana without restrictions.
• At least two years of clinical experience as a nurse in case management or similar health care services.
• Intermediate to advanced computer skills: Word, Excel, Outlook.
• Excellent communication and interpersonal skills with internal/external stakeholders.
• Strong critical thinking, organizational, written and verbal communication.

Responsibilities

• Assess and evaluate member needs across physical, environmental, and psycho-social domains.
• Identify and resolve barriers to effective care.
• Develop longitudinal, trusting relationships to improve care quality and coordination.
• Coordinate all medical and non-medical services for the member.
• Collaborate with ICT and other care team staff.
• Coordinate with Medicare payers/providers for dual-eligible members.
• Serve as primary contact for ICT and external resources.
