Job summary

Humana Healthy Horizons in Indiana is seeking a Care Coordinator 2 (Field Care Manager 2) who assesses member needs and guides families toward appropriate resources to support wellness and independence. This role focuses on Indiana PathWays for Aging, helping Hoosiers age at home with access to services and improved health outcomes.

Qualifications

• Licensed Registered Nurse in Indiana, unrestricted.
• Minimum 2 years clinical nursing experience in case management or similar health care.
• Proficient in Word, Excel, Outlook.
• Exceptional communication and interpersonal skills with internal/external stakeholders.
• Ability to manage multiple priorities in a fast-paced environment.
• Experience with electronic medical records systems.

Responsibilities

• Coordinate and manage member health needs across medical, environmental, and psychosocial domains.
• Develop and maintain longitudinal, trusting relationships with members for better care continuity.
• Coordinate all medical and non-medical services the member requires.
• Collaborate with ICT and care team to address identified needs.
• Coordinate with Medicare payers and providers for eligible members.
