Qualifications

• Licensed RN in Indiana with no restrictions.
• At least two years of clinical experience as a nurse in case management.
• Proficient with MS Word, Excel and Outlook.

Responsibilities

• Coordinate all medical and non-medical services for members, including environmental and psychosocial needs.
• Develop longitudinal, trusting relationships with members to improve care quality and coordination.
• Collaborate with the ICT and care team, and coordinate with Medicare payers and providers.
