Qualifications

• Requires RN license in Indiana.
• 2+ years of clinical nursing/case management experience.
• Proficient MS Word, Excel, Outlook.

Responsibilities

• Coordinate care with member, ICT, and service providers.
• Develop longitudinal, trusting relationships to improve care quality.
• Coordinate all medical and non-medical services as needed.
• Collaborate with Service/Transition Coordinators and care team.
