Job summary

Humana Healthy Horizons in Indiana is seeking a Care Coordinator 2 (Field Care Manager 2) to assess and guide members toward appropriate resources, coordinating with the Interdisciplinary Care Team to support care and benefits. The role requires Indiana RN licensure, strong communication, and the ability to manage multiple priorities in a hybrid field/home setting.

Qualifications

• Licensed RN practicing in Indiana without restrictions.
• Minimum 2 years of clinical case management experience.
• Strong computer skills; proficient with Word, Excel and Outlook.
• Excellent communication and interpersonal skills with diverse stakeholders.
• Strong critical thinking, organizational, written and verbal problem-solving abilities.
• Ability to manage multiple priorities in a fast-paced environment.
• Experience using EMR systems and electronic information tools.
• Residence in Indiana.

Responsibilities

• Assess and evaluate member needs to achieve or maintain optimal wellness.
• Coordinate medical and non-medical services with the care team.
• Develop longitudinal relationships with members to improve care continuity.
• Collaborate with ICT and other staff to address identified needs.
• Coordinate with Medicare payers and providers for beneficiaries eligible for Medicare.
