Job summary

Humana Healthy Horizons Indiana is seeking a Care Coordinator 2 (Field Care Manager 2) to assess and evaluate members' needs, guiding them toward resources and coordinating with the care team to support wellness and independence for Indiana PathWays members.

This role combines remote work with substantial field visits in Indiana, requires an Indiana RN license and 2+ years in case management, and involves coordinating with Medicare payers and service providers to improve access and outcomes.

Qualifications

• Licensed Registered Nurse (RN) in Indiana without restrictions.
• At least two years of clinical experience as a nurse in case management or similar health care services.
• Intermediate to advanced computer skills with Word, Excel, and Outlook.
• Exceptional communication and interpersonal skills with internal and external stakeholders.
• Ability to manage multiple priorities in a fast-paced environment.
• Experience with electronic medical records systems.

Responsibilities

• Facilitate development of longitudinal, trusting relationships with members toward improved quality and coordination of care.
• Coordinate all medical and non-medical services for the member.
• Collaborate with other care team staff to address identified needs.
• Coordinate with Medicare payers, plans, and providers for eligible members.
• Serve as primary contact for the Interdisciplinary Care Team (ICT) and coordinate with members and resources.
