Job summary

CVS Health Corporation is seeking an RN Field Assessor to perform face-to-face assessments and determine member health needs within Morris County, NJ and surrounding counties. The role emphasizes holistic, person-centered care with travel up to 85% and mileage reimbursement.

You will mentor new team members, work Monday–Friday 8:00 AM–5:00 PM EST, and help members access services to improve quality of life.

Qualifications

• Must reside in Morris County, NJ and travel up to 85% of the time.
• Active, unrestricted RN license in New Jersey.
• At least 1 year of clinical experience in hospital, home health, or ambulatory care.
• Strong attention to detail and ability to work independently.

Responsibilities

• Conduct face-to-face assessments to evaluate medical needs and wellness.
• Facilitate access to services such as Adult Medical Daycare and Personal Care.
• Assess and document needs for Nursing Facility Custodial Care and MLTSS enrollment.
• Collaborate with team to overcome barriers and meet goals.
• Develop proactive care plans to improve short- and long-term outcomes.
• Use interviewing skills to identify health needs and plan care.
