Job summary

CVS Health is seeking a compassionate Registered Nurse to conduct assessments and help members access care. This full-time role is based in Trenton, NJ with significant travel and flexible work-from-home elements.

You will perform 10+ weekly assessments, coordinate services such as Adult Medical Daycare and Personal Care Assistance, and mentor new team members. A NJ RN license, NJ Choice Certification, and 1+ year clinical experience are required; residency in Morris County is needed.

Qualifications

• Active, unrestricted RN license (New Jersey required).
• NJ Choice Certification completed.
• 1+ year clinical experience in hospital/home health/ambulatory care.

Responsibilities

• Conduct face-to-face assessments to evaluate medical needs and wellness.
• Help members access services such as Adult Medical Daycare and Personal Care Assistance.
• Complete 10+ assessments per week using a holistic approach.
• Collaborate with team and supervisor to overcome barriers and achieve goals.
• Use interviewing skills to engage members and uncover health needs.
• Mentor new team members after mastering your role.
• Assist with developing proactive plan of care to enhance member outcomes.
• Assess the need for referrals to clinical resources and interdisciplinary team members.

Requirements

Education:

• Associate Degree in Nursing required; BSN preferred.

Experience:

• 1 or more years clinical experience (hospital, home health, or ambulatory care).

Licenses & Certifications:

• Active, unrestricted RN license, New Jersey required.
• Successful completion of the NJ Choice Certification required.
