Job summary

CVS Health is seeking a RN Field Assessor in New Jersey to conduct face-to-face assessments evaluating members' medical needs and overall wellness in a holistic, person-centered approach.

This full-time role requires an active New Jersey RN license, 1+ year of clinical experience, and NJ Choice Certification with at least 80%. Travel up to 85% and residency in NJ counties noted. Training provided to reach full capacity.

Qualifications

• Must hold an RN license in New Jersey.
• 1+ year of clinical experience (hospital, home health, or ambulatory care).
• Active NJ Choice Certification with minimum exam score of 80%.
• Associate Degree in Nursing required; BSN preferred.

Responsibilities

• Conduct face-to-face assessments with members to evaluate medical needs and overall wellness.
• Help members access vital services such as Adult & Pediatric Medical Daycare and Personal Care Assistance.
• Complete 20+ assessments per week using a holistic approach.
• Collaborate with your team and supervisor to overcome barriers and achieve goals.
• Use interviewing skills to engage members and uncover their health needs.
• Mentor new team members once you’ve mastered your role.
• Operate at full capacity by month four with comprehensive training and support.
