Activate Care is seeking a field-based Registered Nurse to travel across Massachusetts, conducting in-person health assessments for a managed care plan's dually eligible members. You will document findings in GuidingCare and hand off to the care management team.

The role emphasizes timely SOAP notes, compliance with HIPAA and NCQA standards, and strict safety protocols. A Massachusetts RN license and 1+ year home health experience are required.

Qualificações

• Active Massachusetts RN license (or higher) in good standing.
• Associate Degree in Nursing required; BSN preferred.
• 1+ years of home health clinical experience required.
• Experience with dual-eligible, Medicaid, or Medicare Managed Care populations preferred.

Responsabilidades

• Conduct In-Person Assessments in the Field across members' homes and community settings.
• Document assessment findings accurately in the health plan’s care management platform (GuidingCare) and send SOAP notes to the Care Manager.
• Submit complete documentation within 48 hours of each assessment and address audit findings within two business days.
• Maintain safety, follow mandated reporting standards, and adhere to HIPAA and NCQA standards.
• Complete ongoing mandatory training to maintain platform access.
