Job summary

Activate Care seeks a licensed Registered Nurse to perform in-home health assessments for dually eligible members across Massachusetts. You will document findings and hand off to care management, with a focus on timely, accurate assessments rather than ongoing care.

The role requires field travel, adherence to HIPAA/NCQA standards, and strong communication with members, families, and the health plan team. Flexible scheduling and field safety are emphasized.

Qualifications

• Active RN license in good standing (MA).
• Minimum 1 year clinical home health experience preferred.
• Experience performing in-home assessments is a plus.
• Ability to document clearly in an electronic system.

Responsibilities

• Travel to members' homes across MA to conduct health assessments.
• Complete required assessments (UCA, FA, Comprehensive); document findings.
• Submit SOAP notes to Care Manager within specified timelines.
• Maintain high quality and complete documentation; adhere to regulatory standards.
