Job summary

Activate Care is seeking a Registered Nurse to travel across Massachusetts, performing in-person health assessments for managed care members. The role focuses on timely documentation and seamless handoff to care management, with training on workflows and privacy standards.

The position requires MA RN licensure, an AS degree in nursing, and at least one year in home health. A valid driver’s license and reliable vehicle are required, with flexible scheduling and potential for sign-on bonus.

Qualifications

• Active RN license in MA in good standing.
• Associate of Science degree in nursing is required; BSN preferred.
• Valid driver's license, personal vehicle and verifiable insurance.
• Minimum of 1 year of clinical home health experience.
• Experience with dual-eligible populations (Medicare/Medicaid) preferred.

Responsibilities

• Engage with members in their homes and community settings to complete assessments.
• Document assessments in GuidingCare per workflows and guidelines.
• Submit 48-hour documentation after each assessment with high completeness.
• Travel to members' homes regularly; adhere to safety and regulatory standards.
• Maintain licensure and certifications; complete required trainings.
