Job summary

Activate Care is seeking a dedicated Registered Nurse to travel across Massachusetts for in-home health assessments for a managed care plan’s dually eligible members. You will perform assessments, document findings, and hand off to care management; this is assessment and documentation, not ongoing care.

Must reside within commutable distance of MA, hold an active MA RN license, and be prepared for up to 50% travel. influenza vaccination is required, and strong documentation skills are essential.

Qualifications

• Active RN licensed in MA in good standing.
• AS degree in nursing required (BSN preferred).
• Valid driver’s license and insured vehicle.
• Minimum of 1 year home health clinical experience.
• Experience with dual-eligible Medicaid/Medicare preferred.
• Assessment/documentation on an electronic platform is required.

Responsibilities

• Engage with members in homes and community settings, respecting cultural and linguistic needs.
• Perform required assessments (UCA, FA, Comprehensive) and evaluate physical, cognitive, and functional status.
• Document assessments in GuidingCare following workflows and guides.
• Submit SOAP notes to Care Manager after assessments.
• Complete documentation within 48 hours of each assessment and maintain quality standards.
• Travel routinely to members’ homes or community settings to conduct assessments.
• Report abuse, neglect, or exploitation as a mandated reporter.
• Adhere to field-safety practices and HIPAA/privacy standards.
