Job summary

Catholic Health is hiring a Weekend RN for Home Care on Long Island. This role covers Babylon, Bayshore, Brightwater, Deer Park, Gilgo Beach, Oak Beach, West Islip, and surrounding areas.

Schedule features 10.5-hour shifts on Saturdays and Sundays, 7:00 AM to 5:30 PM, with eligibility for full-time benefits as a 0.53 FTE. RNs will case manage, assess, plan, and implement home health care plans, measure vitals, administer IVs, and provide both skilled nursing and education to patients and

Qualifications

• New York State RN license is required.
• BLS Certification is required.
• Valid NYS Driver’s License with reliable transportation is required.
• Minimum of one year of hands-on RN experience in home health care or similar setting.

Responsibilities

• Case manages, assesses, plans, develops and implements a comprehensive home health care plan for patients.
• Measure vital signs, administer IVs, change dressings, clean wounds as needed.
• Provide skilled nursing care, preventive rehabilitative procedures, and prescribed treatments for diverse patient populations.

Job Details

Our Home Care RN’s

• Case manages, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the most upmost standard

Minimum Qualifications

• New York State RN license
• Minimum of one year of recent hands-on experience as a Registered Nurse.
• Basic Life Support Certification
• Valid New York State Driver’s License and reliable transportation

Salary Range

USD $100,000.00 - USD $130,000.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
