Job summary

Catholic Health Home Care seeks an experienced Registered Nurse to provide comprehensive home health care services on Long Island. You will assess, plan and implement patient-centered care, collaborating with a multidisciplinary team and educating families on disease management.

Required are an active NY RN license, at least 1 year of RN experience, BLS certification, and a valid NY driver's license. Flexible schedules and travel reimbursement are offered in a supportive work environment.

Qualifications

• New York State RN license is required.
• Minimum of one year of RN hands-on experience.
• Basic Life Support (BLS) certification required.
• Valid New York State Driver’s License with reliable transportation.

Responsibilities

• Case manage, assess, plan, develop and implement home health care plans.
• Measure vital signs and administer IVs; change dressings and wound care as needed.
• Provide skilled nursing care and rehabilitative procedures for diverse patients.
• Collaborate with physicians, therapists, social services and home health aides.
• Educate patients and families on disease management and health promotion.

• 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
• Per visits are required to attend an entire first week of orientation Monday – Friday (8:30AM -4:30PM.)
• Per Visits are required to work 1 shift per pay period.

Job Details

• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the most upmost standard

Minimum Qualifications

Salary Range

USD $62.00 - USD $135.00 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
