Job summary

Catholic Health Home Care, part of a large Long Island health system, seeks a Home Care RN to lead case management and patient care in patients' homes. The role includes shifting hours with rotating weekends, and an orientation period Monday–Friday.

Applicants must hold a New York State RN license, have at least one year of RN experience, BLS, and a valid NY driver's license with reliable transportation. Per-visit pay structure applies, with comprehensive benefits offered.

Qualifications

• Basic Life Support (BLS) certification is required.

Responsibilities

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for patients.
• Measure vital signs, administer IVs, change dressings and cleanse wounds as needed.
• Provide skilled nursing care, rehabilitative procedures and prescribed treatments for diverse patient populations.
• Collaborate with physicians, therapy, social services, pastoral care, aides and others for assigned caseload.
• Educate patients and families on disease management and health promotion as directed by the physician.
• Maintain care to the highest standard of practice.

• 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
• Per visits are required to attend an entire first week of orientation Monday - Friday (8:30AM -4:30PM.)
• Per Visits are required to work 1 shift per pay period.

Job Details

Our Home Care RN’s:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the most upmost standard

Minimum Qualifications

• New York State RN license
• Minimum of one year of recent hands on experience as a Registered Nurse.
• Basic Life Support Certification
• Valid New York State Driver’s License and reliable transportation

Salary Range

USD $62.00 - USD $135.00 /Hr.
