Job summary

Catholic Health Home Care is seeking Registered Nurses to manage and implement comprehensive home health plans in diverse patient settings on Long Island. You will assess, plan, and coordinate care with physicians, therapists, and social services to ensure optimal outcomes.

Ideal candidates hold a NY RN license, have at least one year of hands-on RN experience, BLS certification, and a valid NY driver’s license with reliable transportation.

Qualifications

• New York State RN license.
• Minimum of one year of hands-on RN experience.
• Basic Life Support certification.
• Valid NY driver's license and reliable transportation.

Responsibilities

• Case manage, assess, plan, develop and implement a comprehensive home health care plan.
• Measure vital signs, administer IVs, change dressings, and wound care as needed.
• Provide skilled nursing, rehabilitative procedures, and prescribed treatments for diverse home care situations.
• Collaborate with physicians, therapy, social services, pastoral care, home health aides and others for your caseload.
• Educate patients and families on disease management and health promotion.

Job Details

Our Home Care RN’s:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the most upmost standard

Minimum Qualifications:

• New York State RN license
• Minimum of one year of recent hands on experience as a Registered Nurse.
• Basic Life Support Certification
• Valid New York State Driver’s License and reliable transportation

Salary Range

USD $100,000.00 - USD $130,000.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
