Job summary

Catholic Health Home Care on Long Island seeks an experienced RN for weekend home health assignments. This 0.53 FTE role offers full-time benefits and a stable, supportive team across physicians, therapy, social services, pastoral care, and home health aides.

You will manage caseloads, assess patients, and implement comprehensive home health plans, ensuring timely education and high-quality care consistent with physician plans.

Qualifications

• New York State RN license.
• Minimum of one year of recent hands on experience as a Registered Nurse.
• Basic Life Support Certification.

Responsibilities

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed.
• Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the most upmost standard

Schedule

• Every Saturday & Sunday
• 10.5-hour shifts
• 7:00 AM – 5:30 PM
• Eligible for full-time benefits as a 0.53 FTE

Job Details

• Our Home Care RN’s

Salary Range

USD $100,000.00 - USD $130,000.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
