Job summary

Catholic Health is seeking dedicated Home Care RNs in New York to manage and implement home health care plans for diverse patient populations. Team 1 covers the Northern Nassau Area; Bayville, Bethpage, Carle Place, Great Neck, Hicksville, Kings Point, Lake Success, Manhasset, Mutton Town, Old Bethpage, Port Washington, Roslyn, and Westbury.

• 5 x 8 hour shifts (8:30 AM to 4:30 PM) – rotating weekends

Job Details

Our Home Care RN’s

• Case manage, assess, plan, develop, and implement a comprehensive home health care plan for patients.
• Measure vital signs, administer IVs, change dressings, clean wounds as needed.
• Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the highest standard.

Minimum Qualifications

• New York State RN license.
• Min. 1 year of recent hands‑on experience as a Registered Nurse.
• Basic Life Support certification.
• Valid New York State driver’s license and reliable transportation.

Salary Range

USD $95,000.00 – USD $125,000.00 /yr. This range is an estimate; actual pay will include bonuses, differential pay, and other compensation forms. It does not include the value of benefits.
