Job summary

Catholic Health Home Care on Long Island seeks registered nurses to provide comprehensive in-home care across diverse patient populations. You will manage caseloads, assess needs, and implement treatment plans in patients' homes.

The role requires NY RN license, BLS, and a valid NY driver's license; duties include vitals, IVs, wound care, and collaboration with a multidisciplinary team to promote health and independence.

Responsibilities

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the most upmost standard

• 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends
• Per visits are required to attend an entire first week of orientation Monday – Friday (8:30AM -4:30PM.)
• Per Visits are required to work 1 shift per pay period.

Job Details

Our Home Care RN’s:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.

Minimum Qualifications:

USD $35.00 - USD $135.00 /Hr.
