Family Health Care seeks an RN Complex Care Manager to lead a multidisciplinary, patient-centered care team in Michigan. You will manage high-risk patient groups, coordinate with PCPs, and implement care plans to ensure quality and cost-effective outcomes.

The role includes close collaboration with care teams, infection prevention responsibilities, and educating patients for self-management, with a focus on timely follow-up and resource utilization.

Qualifications

• Current RN licensure in Michigan.
• Associates degree in nursing required; BSN preferred.
• Minimum 5 years clinical nursing experience.
• Knowledge of provider community and resources in White Cloud area.
• Comprehensive knowledge of insurance concepts and outpatient delivery systems.

Responsibilities

• Coordinate care for patients identified using MiPCT guidelines.
• Manage a caseload of ~150 complex patients with 30–50 actively engaged.
• Assess health, education, and psychosocial needs using standardized tools.
• Collaborate with PCP to develop/update a comprehensive care plan in NextGen template.
• Provide targeted interventions to avoid hospitalizations/ER visits.
• Coordinate care across settings and assist patients/families in understanding health options.
• Provide self-management support to empower patient self-care.
• Infection control oversight and adherence to CDC guidelines.
