Job summary

Qualifications

• Must be a Registered Nurse with MI licensure and RN degree.
• Experience in adult medicine or primary care is preferred.
• Lifesaving CPR certification may be required.

Responsibilities

• Develop, implement and monitor individualized care plans for high-risk patients.
• Collaborate with care team and community resources to coordinate care.
• Provide patient self-management support and education.
• Document timely follow-ups, medication reconciliation and care transitions.
• Identify high-risk populations and adjust plans based on risk stratification.
