Job summary

Qualifications

• RN licensed to practice in Michigan.
• Three (3) years clinical nursing experience serving chronically ill patients.
• Understanding of Population Health Management and industry trends.
• Familiar with initiatives of managed care health plans.
• Ability to work self-directed and with a team.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provides telephonic and face-to-face comprehensive assessment and care management services.
• Uses assessment tools to target high risk and vulnerable populations.
• Assesses health, educational, and psychosocial needs over time.
