Qualifications

Responsibilities

• Perform thorough patient assessments and develop care plans with patient-family involvement during discharge and transitions.
• Aggregate and report data on patient outcomes and resource use for committees and stakeholders.
• Ensure continuity of care and discharge planning for complex patients per regulatory standards.
• Provide clinical mentorship to frontline care management staff to improve care quality and safety.
• Advocate for patients and families to optimize hospital resources and adhere to guidelines.
• Initiate referrals and document discharge planning interventions per standards.
• Lead multidisciplinary rounds and collaborate with teams to resolve barriers to discharge and care coordination.
• Engage with physician practices and leaders to ensure efficient progression of care and compliance with regulations.
• Develop relationships with community agencies to support clinically complex patients.
• Demonstrate age-appropriate care knowledge and apply department policies to patient needs.
