Job summary

MyMichigan Health is seeking an RN Case Manager to maintain high-quality patient care during medical centralization and post-discharge while ensuring full reimbursement. You will plan, execute, and monitor care across the continuum, educate physicians and teams on medical necessity, and ensure timely transitions in care to reduce readmissions.

Qualifications

• Three to five years clinical experience.
• Certification in case management preferred.
• ACM certification recommended after two years.

Responsibilities

• Work with healthcare team, patient and family to transition care to appropriate level.
• Collaborate with care management team to discuss admission, discharge criteria and reimbursement fundamentals.
• Lead daily care coordination meetings and identify high-risk factors.
• Facilitate discharge planning and home care arrangements.
• Analyze admission data to ensure quality care and appropriate location.
