Quartz Health Solutions seeks an RN Complex Care Specialist to lead care management for high-acuity members in Wisconsin. You will conduct clinical assessments, design individualized care plans, and collaborate with providers and families to optimize outcomes.

You will evaluate self-management skills, address barriers, and apply shared decision‑making. Proficiency with EHRs and Epic is valued, with opportunities for professional growth and advancement.

Qualifications

• Active WI RN license or compact state license.
• Bachelor's or Associate degree in Nursing.
• 3+ years in clinical nursing, case management, or care coordination.
• Experience with complex care or chronic disease management preferred.

Responsibilities

• Conduct thorough assessments to determine health objectives and obstacles to care.
• Develop and implement individualized care plans with the care team.
• Act as an advocate, resource, and educator for members and families.
• Leverage chronic disease management concepts to support self-management.
• Coordinate care transitions and ensure continuity across providers and settings.
• Adhere to NCQA standards for outreach, care plans, and assessments.
