You will contribute to care planning, coordinate with interdisciplinary staff, and support transitions while maintaining dignity, comfort, and independence for residents.

Qualifikationen

• Current Michigan Registered Nurse (RN) license.
• Graduation from an accredited nursing program.
• Current CPR certification or willingness to obtain CPR certification.
• Nursing experience in skilled nursing, home health, medical-surgical, critical care, or related settings preferred.
• Strong clinical assessment, communication, and documentation skills.
• Commitment to resident-centered care and teamwork.

Aufgaben

• Provide skilled nursing care tailored to each resident's needs and preferences.
• Collaborate with physicians, nursing assistants, therapists, social workers, dietitians, and other team members to ensure coordinated care.
• Assess resident conditions, monitor changes, and implement orders while advocating for interventions.
• Mentor nursing assistants and guide team members to uphold professionalism and resident-centered care.
• Contribute to care planning, admissions, discharges, and transitions.
• Support families, coordinate with physicians, and respond to changes promptly.
