Job summary

Wellspring Lutheran Services in Frankenmuth, Michigan is seeking a Licensed Practical Nurse (LPN) to join a compassionate care team. The role emphasizes person-centered care, building meaningful relationships with residents and families, and working collaboratively with an interdisciplinary team to support health, comfort, and quality of life.

You will provide hands-on nursing, advocate for residents, monitor changes, and participate in admissions, care planning, and daily assessments to ensure

Qualifications

• Current Michigan Licensed Practical Nurse (LPN) license.
• Graduation from an accredited Practical Nursing program.
• Current CPR certification or willingness to obtain CPR certification.
• Nursing experience in skilled nursing, home health, medical-surgical, critical care, or related settings preferred.
• Strong clinical, assessment, communication, and documentation skills.
• Commitment to compassionate, resident-centered care.

Responsibilities

• Provide hands-on nursing care to residents, addressing physical, emotional, and social needs.
• Get to know residents to tailor care to personal preferences and goals.
• Advocate for residents while coordinating with physicians, assistants, therapists and the care team.
• Monitor condition changes, communicate concerns, implement physician orders, and ensure timely interventions.
• Guide and support nursing assistants during shifts to foster teamwork and quality care.
• Contribute to admissions, discharges, care planning, and ongoing assessments to maintain a safe, welcoming environment.
