Job summary

HCA Houston Healthcare Southeast in Pasadena, TX seeks a Skilled Nursing Facility Clinical Nurse Coordinator (RN) to lead the post-acute unit, oversee nursing care, and coordinate patient flow from admission to discharge. The CNC collaborates with therapists, supports hourly rounds, mentors staff, and drives initiatives to improve outcomes.

Requires Texas RN license, BLS, and SNF experience; BSN preferred. We offer comprehensive benefits and a strong leadership team within a 340+ bed hospital,

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Current Texas RN license or Multi-State Compact; 90-day license transfer if needed.
• Current BLS certification required.
• 1 year SNF clinical experience as an RN in an acute care setting required.

Responsibilities

• Assist with admission and discharge processes to ensure efficient unit throughput and high-quality, patient-centered care.
• Manage staff scheduling and labor productivity per patient care needs and guidelines.
• Partner with management and subject matter experts to evaluate patient care services and foster teamwork.
• Operationalize nursing initiatives, including hourly rounds, employee rounding, and service recovery.
• Provide recommendations for interviewing, hiring, development plans, and performance evaluations.
• Maintain department inventory control and active management of supplies and equipment.
