Option Care Health is seeking a Clinical Transition Educator in Colorado Springs to provide clinical education and training, ensuring patients transition smoothly to our home infusion delivery model.

The role partners with the sales team to grow patient transitions, requires an RN license and at least two years in healthcare, and involves 100% local travel to care facilities and hospitals. Flexible part-time hours and comprehensive benefits are provided.

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years of healthcare experience.
• Excellent relationship-building and customer-service skills.
• Strong PC skills including Word, Excel, and PowerPoint.
• Willingness to travel locally to care facilities and hospitals.

Responsibilities

• Educate and train patients, caregivers, and staff on OCH services for home/in‑home delivery.
• Assess patient viability for the OCH care delivery model.
• Coordinate with sales to align clinical education with growth targets.
• Maintain confidentiality and follow HIPAA guidelines.
• Document education using OCH applications and technology.
• Facilitate smooth transition from hospital to home or alternate settings.
• Provide in-person patient and caregiver education and collect admission/insurance information.
