Job summary

Option Care Health is seeking a Clinical Transition Educator (RN) in Colorado Springs for a part-time role. The position focuses on delivering clinical education to ensure smooth transitions of patients to our care model, while partnering with the sales team to grow transition volumes.

The Educator will educate patients, caregivers, and facilities, perform patient assessments, and coordinate hospital-to-home transitions, with travel to care sites and hospitals as required.

Qualifications

• Active and unrestricted RN license required.
• Minimum 2 years of healthcare experience.
• Experience establishing relationships with individuals at all levels of the organization.
• Experience applying knowledge of industry standards (ACHC, URAC).
• Customer service experience to internal and external customers.
• Basic PC skills: internet, email, MS Word, Excel, PowerPoint.
• Willingness to travel locally to care facilities and hospitals.

Responsibilities

• Educate, train patients, caregivers, and facility staff about how OCH services will be delivered in home, alternate sites, or virtually.
• Conduct patient assessments to determine eligibility for the OCH model.
• Partner with the sales team to understand educational needs and improve patient transition volume.
• Maintain confidentiality of patient information and comply with regulations.
• Document education using OCH applications and technology.
• Coordinate transition from hospital to home with facility staff to ensure timely discharge and continuity of care.
• Provide in-person education and gather pre-admission and insurance information for service initiation.
