Option Care Health is hiring a Clinical Transition Educator in Connecticut to lead clinical education for successful patient transitions to our home delivery model. You will partner with the sales team to drive patient transitions and ensure high-quality care at home.

The role requires RN licensure, 2+ years in healthcare, and strong interpersonal and PC skills. Travel is local to care facilities and hospitals, with comprehensive benefits and growth opportunities.

Qualifications

• Active and unrestricted RN license required.
• Minimum 2 years of healthcare industry experience.
• Ability to establish relationships with individuals at all levels of an organization.
• Knowledge of ACHC/URAC standards is a plus.
• Customer service experience and strong communication skills.

Responsabilités

• Educate patients, caregivers and staff about OCH services at home or alternate sites.
• Assess patient viability for transition to OCH care delivery.
• Coordinate with sales to address educational needs and improve transition volume.
• Maintain confidentiality of patient and company information.
• Document education activities using OCH applications and technology.
• Coordinate hospital-to-home transitions with facilities and teams.
• Provide in-person education and gather insurance information for service initiation.
