Transitions At Home is seeking a Full-Time RN Clinical Manager to provide clinical leadership within our Home Health team in Wisconsin. You will support nursing and therapy staff and ensure high quality patient care in home settings.

Ideal candidates have 3-5 years of nursing experience (1 year in Home Health preferred), CPR certification, and a Wisconsin RN license. A company car, benefits, and growth opportunities are included.

Transitions At Home, is currently seeking to hire a Full-Time, experienced and compassionate RN Clinical Manager to join our Home Health team! This position provides clinical leadership and support to our nursing and therapy staff while ensuring quality patient care and compliance with all applicable standards. What does a Transitions at Home RN Clinical Manager do?
Transitions At Home RN Clinical Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient's physician orders and plan of care under the direction of the Administrator.

Hours: Full-Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• Limited on-call rotation

Qualifications / Job Requirements:

• Active Wisconsin Registered Nurse License
• Three to five (3-5) years' nursing experience, 1 year in Home Health (preferred)
• Ability to supervise, guide and develop nursing skills and performances
• Participates in the development of standards which ensure safe and therapeutically effective service to patient/family's.
• CPR Certification
• A valid Wisconsin Driver's License and Car Insurance
