Job summary

Transitions At Home for Northern Wisconsin is seeking an experienced RN Case Manager to support our care team and coordinate patient-centered care in patients' homes. This role offers a full-time schedule with a Monday–Friday day shift and no nights or weekends.

Responsibilities include assessments, medications, treatments, and coordinating services while counseling patients and families. Competitive wage, company car with gas card, and strong benefits are provided to eligible employees.

Qualifications

• Active Wisconsin Registered Nurse License.
• Two years of nursing experience, with 1 year in Home Health preferred.
• CPR Certification is required.
• A valid Wisconsin Driver's License and car insurance.

Responsibilities

• Provide skilled nursing care including assessments, medications, and treatments.
• Coordinate services and counsel patient and family in home per physician orders.
• Work under RN Clinical Manager direction to deliver patient-centered care.

What does a Transitions at Home RN Case Manager do?

Transitions At Home Registered Nurse Case Managersprovide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full-Time

• Starting at 30 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Nights or Weekends
• Limited on-call rotation
