Transitions At Home in Wisconsin is seeking a full-time Administrative Assistant to support our Home Health team. The role emphasizes organized, dependable, and professional front-office operations, processing referrals and patient information accurately and timely.

You will handle EMR data entry, manage communications with hospitals and clinics, answer calls, and maintain confidentiality in a fast-paced healthcare office. Prior medical office experience strongly preferred.

Qualifications

  • Medical Assistant degree, diploma, or certificate required.
  • Healthcare-related education (e.g., Medical Office Administration, Health Information Technology) will be considered.
  • Experience with EMR/EHR systems preferred.
  • Strong organizational and data-entry skills required.
  • Professional telephone and customer service skills expected.

Responsibilities

  • Receive, process, and track incoming home health referrals.
  • Enter patient demographics, insurance, physician, and referral info into EMR.
  • Review referrals for completeness and follow up for missing docs.
  • Maintain accurate electronic medical records and file documents.
  • Communicate with hospitals, physician offices, clinics, and referral sources.
  • Answer calls and provide receptionist/front-office support.

Job description

Transitions At Home, is currently seeking to hire a full-time Administrative Assistant. We are seeking an organized, dependable, and professional Administrative Assistant to join our Home Health team. This position plays an important role in supporting daily agency operations and ensuring referrals and patient information are processed accurately and timely.


Key Responsibilities



  • Receive, process, and track incoming home health referrals.

  • Enter new patient demographic, insurance, physician, and referral information into the electronic medical record (EMR).

  • Review referrals for completeness and follow up for missing documentation.

  • Maintain accurate and organized electronic medical records.

  • Scan, upload, file, and retrieve patient documentation.

  • Communicate with hospitals, physician offices, clinics, skilled nursing facilities, patients, families, and other referral sources.

  • Answer incoming telephone calls and provide professional receptionist/front-office support.

  • Route clinical questions and concerns to appropriate clinical staff.

  • Process incoming faxes, medical records, orders, and other correspondence.

  • Assist clinical and administrative staff with daily office operations.

  • Maintain patient confidentiality and comply with HIPAA requirements.

  • Perform additional administrative and office duties as assigned.


Hours: Full-Time Location: On-Site



  • 40 hours per week 8:00 am - 4:30pm

  • Monday through Friday

  • No Holidays or Weekends

What We’re Looking For:


We are looking for someone who is organized, detail-oriented, self-motivated, and comfortable working in a busy healthcare environment. The successful candidate will communicate effectively with patients, healthcare providers, referral sources, and team members while helping ensure smooth and efficient office operations.


Home health experience is a plus, but we are willing to consider candidates with strong medical office or healthcare administrative experience.


Qualifications / Job Requirements:



  • Medical Assistant degree, diploma, or certificate required.

  • Other healthcare-related education such as Medical Office Administration, Health Information Technology, Medical Billing/Coding, Healthcare Administration, or similar education will be considered.

  • Previous medical office, home health, hospice, hospital, clinic, or other healthcare experience preferred.

  • Knowledge of medical terminology is preferred.

  • Experience with electronic medical records (EMR/EHR) preferred.

  • Strong computer and data-entry skills.

  • Excellent organizational skills and attention to detail.

  • Professional telephone and customer service skills.

  • Ability to multitask and prioritize in a fast-paced healthcare environment.

  • Must maintain confidentiality and demonstrate professionalism and dependability.
