You will work with a compassionate team in a supportive, family-style work culture. The role includes assessments, medications, treatments, and planning of services in the patient’s residence, under supervision of the RN Clinical Manager and Administrator.

Qualifications

• Active Wisconsin Registered Nurse license is required.
• Two years of nursing experience, with one year in home health preferred.
• CPR certification is required.
• A valid Wisconsin driver's license with car insurance is preferred.

Responsibilities

• Provide skilled nursing care, including assessments, medications, and treatments in the home.
• Coordinate services and counsel patients and families in the residence.
• Plan and oversee patient care per physician orders and plan of care.
• Work under the direction of the RN Clinical Manager and Administrator.

We're hiring! Transitions At Home, is currently seeking to hire a Full-Time RN Case Manager in the Madison area. Our growing home health agency is seeking a compassionate and experienced full-time RN Case Manager to join our team. In this role, you’ll coordinate and oversee patient care in the home setting, ensuring personalized, high-quality outcomes for every client.

What does a Transitions at Home RN Case Manager do?

Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager and Administrator.

Hours: Full-Time

Paid Drive Time - Company Vehicle

• Monday through Friday 8am-430pm
• No Weekends or Holidays
• Limited on-call rotation
