Transitions At Home is seeking an experienced RN Clinical Manager to lead our Home Health team in Wisconsin. This full-time role provides clinical leadership, supports nursing and therapy staff, and ensures high-quality patient care while complying with standards.

The RN Clinical Manager will assess patients, coordinate services, and counsel families in the patient’s home under physician orders. A strong leader with home health experience is preferred and a flexible schedule is offered.

Qualifications

• Active Wisconsin Registered Nurse License.
• 3–5 years nursing experience; 1 year in Home Health preferred.
• CPR certification required.
• Valid Wisconsin driver's license and car insurance.

Responsibilities

• Provide skilled nursing care including assessments, medications, and treatments.
• Coordinate services and counsel patient and family in their home per physician orders and plan of care.
• Lead and supervise nursing staff under the Administrator's guidance.

Transitions At Home, is currently seeking to hire a Full-Time, experienced and compassionate RN Clinical Manager to join our Home Health team! This position provides clinical leadership and support to our nursing and therapy staff while ensuring quality patient care and compliance with all applicable standards. What does a Transitions at Home RN Clinical Manager do?

Transitions At Home RN Clinical Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction of the Administrator.

Hours: Full-Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• Limited on-call rotation

Qualifications / Job Requirements

• Active Wisconsin Registered Nurse License
• Three to five (3-5) years’ nursing experience, 1 year in Home Health (preferred)
• Ability to supervise, guide and develop nursing skills and performances
• Participates in the development of standards which ensure safe and therapeutically effective service to patient/family's.
• CPR Certification
• A valid Wisconsin Driver's License and Car Insurance
