The position is full‑time, with standard weekday hours (no weekends or major holidays) and a focus on independence within a supportive team.

Qualifications

• Active Wisconsin Registered Nurse License is required.
• Two (2) years' nursing experience, 1 year in Home Health preferred.
• CPR Certification is required.

Responsibilities

• Provide expert guidance and oversight in clinical and regulatory matters.
• Coordinate services and counsel patients and families in the home per physician orders and plan of care under supervision of the RN Clinical Manager.

What does a Transitions at Home RN Case Manager do?

Transition at Home RN will provide expert guidance and oversight in clinical and regulatory matters. This role includes staff training, critical thinking, care plan review, incident investigation, and Who also coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full - Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Weekends or Holidays
• Limited on-call rotation
