Job summary

Transitions At Home for Northern Wisconsin seeks an experienced RN Case Manager to support our care team and deliver patient-centered care in the home. The role emphasizes assessment, medication administration, and coordination of services in patients' residences under supervision.

The position offers a 20–29 hour workweek with a 8:00 am–4:30 pm schedule, Monday through Friday, no nights or weekends, and limited on-call. Competitive wages and a strong benefits package accompany the role.

Qualifications

• Active Wisconsin RN license required.
• Two years nursing experience; 1 year in Home Health preferred.
• CPR certification required.
• Valid Wisconsin driver's license and car insurance.

Responsibilities

• Provide skilled nursing care, including assessments, meds and treatments.
• Coordinate services and counsel patient/family in home setting per physician orders.
• Work under RN Clinical Manager supervision to ensure plan of care.

What does a Transitions at Home RN Case Manager do?

Transitions At Home Registered Nurse Case Managersprovide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Part-Time

• 20-29 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Nights or Weekends
• Limited on-call rotation
