Transitions At Home for Southern Wisconsin is seeking an experienced RN Case Manager to coordinate and deliver patient-centered care in patients' homes. The role emphasizes clinical oversight, care plan review, and collaboration with the care team.

The RN will coordinate services, counsel patients and families in the residence, and support the RN Clinical Manager to ensure regulatory compliance and quality care.

Qualifications

• Active Wisconsin Registered Nurse License.
• Two (2) years nursing experience, 1 year in Home Health preferred.
• CPR Certification.

Responsibilities

• Provide guidance and oversight in clinical and regulatory matters.
• Coordinate services and counsel patient and family in their home.
• Perform care plan review and incident investigation under RN Clinical Manager.

What does a Transitions at Home RN Case Manager do?

Transition at Home RN will provide expert guidance and oversight in clinical and regulatory matters. This role includes staff training, critical thinking, care plan review, incident investigation, and Who also coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full - Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• Limited on-call rotation
