Transitions At Home, a Wisconsin-based nonprofit home health organization, seeks a full-time Registered Nurse Case Manager to coordinate and oversee patient care in the home setting near Sun Prairie. You will assess, plan, implement, and monitor care under the RN Clinical Manager and Administrator.

The role offers a Monday–Friday schedule, a company vehicle with all expenses paid, comprehensive benefits, 401(k) with match, tuition reimbursement, and paid time off.

Qualificações

• Active Wisconsin Registered Nurse License.
• Two years nursing experience, with one year in Home Health preferred.
• CPR Certification is required.
• A valid Wisconsin Driver's License and Car Insurance.

We're hiring! Transitions At Home, is currently seeking to hire a Full-Time RN Case Manager in the Sun Prairie area. Our growing home health agency is seeking a compassionate and experienced full-time RN Case Manager to join our team. In this role, you’ll coordinate and oversee patient care in the home setting, ensuring personalized, high-quality outcomes for every client.

What does a Transitions at Home RN Case Manager do?

Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager and Administrator.

Hours: Full-Time

Paid Drive Time - Company Vehicle

• Monday through Friday 8am-430pm
• No Weekends or Holidays
• Limited on-call rotation
