Job summary

Option Care Health is seeking a Clinical Care Transition Specialist / RN in Hackensack, NJ. This role focuses on educating patients and facilities about home infusion services, ensuring proper patient placements, and growing referral partnerships.

The position requires an active RN or Pharmacist license and at least two years in healthcare. Travel to care facilities and hospitals is expected, with strong communication to patients, families, and providers to support timely discharges.

Qualifications

• Licensed RN or Pharmacist with an active license in the state of practice.
• Minimum 2 years of healthcare industry experience.
• Strong relationship-building and customer-service skills.
• Proficient PC skills: internet, email, Word, Excel, PowerPoint.
• Willingness to travel to care facilities and hospitals.
• Excellent communication with providers, patients, and families.

Responsibilities

• Proactively maintains and grows referrals sources to increase sales and patient starts.
• Educates patients, families, and facility staff about home infusion services.
• Assess patient suitability for alternative site care and coordinate transitions.
• Collaborates with intake, case managers, and discharge planners to ensure quality service.
• Support transfer of orders via phone, fax, and e-prescribing.
• Provide hands-on, in-person education to patients and caregivers for timely discharge.
• Represent Option Care to referral sources and clinicians.
