Option Care Health seeks a Clinical Care Transition Specialist / RN to educate patients and teams about home infusion services, driving seamless transitions to home or alternate sites. The role emphasizes building referral partnerships, coordinating discharge plans, and timely patient starts across facilities in Mount Vernon, IL and surrounding areas.

You will travel to care facilities, hospitals, and sites while collaborating with account managers to meet sales and service goals.

Qualifications

• RN or licensed Pharmacist in the practice state with 2+ years in healthcare.
• Proven ability to establish and maintain relationships at all levels.
• Excellent customer service, communication and planning skills.

Responsibilities

• Proactively grows relationships with referral sources to increase sales and starts.
• Educates patients, families, and staff about home infusion services.
• Coordinate care transitions and ensure timely discharge to home or ATS.
• Collaborate with account managers to execute area growth plans.
• Review medical records and insurance information for pre-admission and referrals.
