Option Care Health is seeking a Clinical Transition Specialist / RN for Hayward, CA. This full-time role focuses on educating patients and families, coordinating with discharge planners, and driving sales through strong relationships with referral sources.

Travel to care facilities and hospitals is required. The position requires a licensed RN in California with at least 2 years in healthcare, excellent communication, and proficiency with MS Office.

Formación

• Licensed Registered Nurse (RN) in the state of practice with at least 2 years in healthcare.
• Experience establishing relationships with individuals at all levels of the organization.
• Experience applying knowledge of standard practices and current applicable standards.
• Experience providing customer service to internal and external customers.
• Basic PC skills: proficient with internet, email, MS Word/Excel/PowerPoint.

Responsabilidades

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with area service providers to sell Option Care services.
• Conducts assessment of patients to determine candidacy for alternative site care.
• Educates patients, families and facility staff about services and products.
• Provides hands-on in-person education to patients and caregivers for timely discharge and independence.
• Communicates with agency staff, medical team, patients and family during discharge to implement treatment plan.
• Collects and reviews pre-admission information and secures signoffs.
• Coordinates with Account Manager and Regional Sales Director on area business and growth plans.
• Delivers educational and promotional programs to patient/caregiver and referral sources with marketing staff.
• Reviews medical records for history and insurance payor information; informs patient registration.
• Initiates care transition with referral sources and internal partners for seamless home care.
• Supports data collection for therapy start and patient tracking; transfers orders via phone, fax, or e-prescribing.
• Maintains confidentiality of patient and proprietary information.
• Provides oversight to providers regarding patient assessment and transition to home care.
• Acts as a centralized contact with other providers.
• Arranges special medical supplies or appliances as needed.
