The position is field-based with a home-based office; travel up to 15% and a flexible schedule Monday–Friday, 8 am–5 pm.

Qualifications

• Current, unrestricted RN licensure is required.
• 4+ years of clinical work experience.
• 3+ years managing needs of complex populations (e.g., Medicare/Medicaid).
• 1+ years of community case management experience coordinating care.
• Ability to travel up to 15% of time.

Responsibilities

• Serve as primary care manager for members with complex medical needs.
• Engage members via multiple modalities to assess medical, behavioral, functional, and social needs.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, and social determinants of health.
• Collaborate with internal teams, providers, and community resources to execute care plans.
• Monitor and update care plans and educate on self-management and lifestyle changes.
• Coordinate discharge planning and care transitions after ER, inpatient, or SNF stays.
