• Ability to travel approximately 15% of the time.

Responsibilities

• Serve as primary care manager for members with complex medical needs.
• Engage members via in-person or telephonic visits to perform comprehensive needs assessments.
• Develop and implement person-centered care plans aligned with guidelines.
• Coordinate with internal teams, providers, and community resources.
• Monitor and update care plans; address changing conditions and assess outcomes.
