Job summary

CareSource is seeking a Clinical Care Manager for Massachusetts to provide comprehensive care management for dually-eligible enrollees. In this mobile role, you will engage with enrollees in their homes, ensuring their medical and social needs are met through effective communication and resource coordination.

This position requires a nursing degree or a Master's in social work, alongside relevant experience in complex care management. The salary range is $90,000 to $120,000, complemented by a bonus and a comprehensive benefits package.

Qualifications

• Current unrestricted clinical license in Massachusetts as RN, LCSW, LISCW, or LMHC required.
• Three years of experience as a Registered Nurse/BH Clinician or one year plus two years of experience working with complex needs.
• Medicaid and/or Medicare managed care experience preferred.

Responsibilities

• Engage with enrollees in homes to establish effective care management relationships.
• Function as a liaison between healthcare providers, community resources, and enrollees.
• Lead interdisciplinary care team and collaborate to create holistic care plans.
• Assist members in accessing community resources and educate them about their benefits.
