Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts. This home-visit role supports dually-eligible enrollees with complex medical, behavioral, and social needs, coordinating care and resources to improve outcomes.

You will lead the interdisciplinary team, develop care plans, and ensure effective communication among providers, patients, and families—and you will educate members about benefits and preventive care. This is a mobile position requiring travel.

Qualifications

• ASN in Nursing from an accredited program.
• Active RN license in Massachusetts required.
• Experience in care coordination or case management preferred.
• Strong interpersonal and communication skills; culturally competent.

Responsibilities

• Engage enrollees in homes and community settings for complex care management.
• Lead the ICT to create holistic plans addressing medical and social needs.
• Coordinate with providers, community resources, and state agencies.
• Educate members on benefits, chronic disease management, and preventive care.
• Extensive travel within the assigned territory; ensure seamless care transitions.
