Job summary

Maximus is seeking a Registered Nurse, Clinical Assessor to join our team and conduct person-centered assessments in homes, group homes, and care facilities to determine Long Term Care needs and placement options.

Travel within and coverage of Washington County, Maine, is required. This role supports the State of Maine's OADS program and involves on-site, video, or telephonic assessments based on contract requirements.

Qualifications

• Active Registered Nurse (RN) license.
• Experience or familiarity with long-term care or home health settings preferred.
• Experience with OASIS or MDS preferred.

Responsibilities

• Conduct person-centered medical and behavioral health assessments per contract guidelines.
• Perform on-site, video, or telephonic assessments to determine needs and level of care.
• Ensure HIPAA-compliant documentation and accurate record-keeping.
• Travel to client locations to perform in-person assessments for majority of the workweek.
